Yizkor Booklet 2025

The Yizkor service includes a pledge to give tzedakah in memory of departed loved ones. Since 2020, we have prepared an annual Yizkor Booklet to honor those who have passed, listing the names of relatives and friends to be remembered during Yizkor services.

The booklet will be distributed at the Yom Kippur Yizkor service and used again during other Yizkor services throughout the year. 

To include names in the 2025 booklet, please fill out the form below.

• $36 for the first name
• $18 for each additional name
• Additional donations are always welcome.

If you would like to repeat last year’s listings, simply clcik “I would like to keep last year’s entry” from the options below and submit your payment—no need to re-enter each name. Copy of last year's Yizkor book can be found here.

If you would like to purchase a Yahrzeit Plaque for the sanctuary in memory of a loved one, please complete the separate form linked below. All plaque purchases include a listing in this year’s Yizkor Book.

Submission deadline:
Please submit all entries by Sunday, September 21, 2025, at 11:00 PM, so we have ample time to prepare the booklet.
